Fill a PDF with Squarespace Form Submissions Using PDF.co and Zapier

Step 1: Prepare Your Squarespace Form

Use a Squarespace form block on a plan that supports the Zapier integration. In the form’s storage settings, connect Zapier.

Create a Squarespace API key with Forms permissions to connect your site to Zapier. See the Squarespace Forms connection guide for requirements and setup details.

Step 2: Prepare the PDF Template

Use a PDF containing interactive form fields. Open it in PDF Inspector in your PDF.co account to identify the exact field names.

Upload the template to PDF.co’s built-in file storage or another location accessible to PDF.co, then copy its file link.

Step 3: Set Up the Trigger

Create a Zap and select Squarespace Forms → New Form Submission.

Connect your Squarespace account using the API key, select your form, and test the trigger.

Squarespace may provide generated sample values during testing. Use the returned fields to configure your mappings, then verify the workflow with a real submission after publishing.

Step 4: Configure PDF.co

Add PDF.co → PDF Filler.

Connect your PDF.co account using OAuth by signing in and authorizing Zapier.

Configure:

• Source PDF: Enter your template’s file link.
• Output PDF Name: Enter completed-form.pdf, or build a filename using submission data.
• Fillable Form Fields: Enter mappings in the format page;fieldName;value, separated by |.

For example:

0;FullName;[Squarespace Name]|0;Email;[Squarespace Email]

Replace FullName and Email with the exact field names from your PDF. Replace the bracketed placeholders with mapped Squarespace values using Zapier’s field picker.

Test the action and open the returned url to check the filled form.

Step 5: Save the PDF to Google Drive

Select the destination drive and folder, then map the PDF.co output url to File.

If Convert to Document is available, set it to False to preserve the PDF format. This setting does not flatten the PDF or make its form fields non-editable.

Test the upload and confirm that the completed PDF appears in your selected folder.

Step 6: Publish and Verify

Publish the Zap and submit a new response through your Squarespace form.

Check that the completed PDF contains the submitted values and has been saved to Google Drive.
